Community Navigator-Re-entry serves as a guide for incarcerated
offenders as they transition to life in the community after their
release. This job operates in a variety of environments. The
employee in this role will meet with people supported in a variety
of locations, such as coffee shops, Rise office, person supported's
home, etc. The employee will also meet with prospective and current
employers of persons supported. ResponsibilitiesTransition Planning
- work with Department of Correction (DOC) transition staff to
create person-centered release plans that include the actions
individuals can take pre-release and post-release.Benefit
Navigation and Enrollment - determine enrollment eligibility for a
variety of benefits; ensure Medical Assistance paperwork is ready
to submit the day of release.Building a Support Team - identify an
individual's needs for success and for meeting the requirements of
their release; line up referrals to the proper supports, such as
psychiatric support; finding an AA, NA, or Anger Management group
in their county; identifying an employment counselor, a mentor,
housing services, etc.Creating a Resource Network - develop
expertise in support services available across Central Minnesota by
performing outreach and building relationships.Communication -
Speak clearly; listen to others; write effectively; establish
relationships; act with integrity; establish trustResponsible for
maintaining a caseload of 15-20 people while tracking and
documenting variety of data about services provided as well as
billing through electronic case management systems.Must be able to
travel to various locations throughout the day in the Twin Cities
and Central MN area.Requirements:Degree in human service field and
experience working with adults with a mental health diagnosis,
providing mental health servicesExcellent time management and
multi-tasking skills with strong ability to problem solveTechnology
skills: Proficient with Microsoft Office software (Outlook, Word,
etc.) Case management (Care Director) - Learn software system and
become proficientCurrent, valid driver's licenseA driving record
that demonstrates safe, legal driving habitsCompliance with Rule 11
and MVR background check requiredPrevious experience practicing
person-centered practices with ex-offenders preferredKnowledge of
housing resources, mental health supports, and other resources
necessary for successful re-entry preferred BenefitsWage: $19.50 -
$20.50/hr depending on qualificationsDaytime work hours (No
