Nursing Home provides care for up to 65 residents who experience a
variety of physical and behavioral health issues. Position:
Registered Nurse Clinical Nurse Manager The RN clinical nurse
manager provides nursing care that meets individualized resident
needs, using recognized nursing techniques, procedures and
established standards based on the Minnesota Board of Nursing scope
of practice. This position functions within and is supportive of
the goals and objectives of Mission Nursing Home and the nursing
department. Conduct consistently reflects the facility’s mission
and values, supporting a culture of respect, accountability, and
compassion in the delivery of resident care and in collaboration
with colleagues. Education and Experience Minimum of 2–3 years of
nursing experience in long-term care, skilled nursing, or a related
healthcare setting. Prior supervisory or nurse management
experience preferred. Knowledge of state/federal long-term care
regulations and standards of practice. Strong leadership,
communication, and critical thinking skills. Ability to collaborate
effectively with residents, families, staff, and interdisciplinary
team members. Proficiency with electronic health records (EHR)
systems. Qualifications, Licensing/Certifications Must possess a
current, unencumbered, active license to practice as an RN in
Minnesota. Essential Job Duties and Responsibilities Plan,
organize, and oversee daily nursing operations to ensure the
delivery of safe, high-quality care. Supervise, support, and
evaluate nursing staff to promote professional growth and
accountability. Conduct regular rounds to monitor resident care and
ensure compliance with care plans and physician orders. Assist in
the development, implementation, and evaluation of resident care
plans in collaboration with the interdisciplinary team. Ensure
compliance with state and federal regulations, facility policies,
and nursing standards of practice. Monitor and maintain accurate
clinical records, assessments, documentation, and reporting
requirements. Supervises licensed nurses and other nursing
personnel in the delivery of care and services according to each
residents plan of care. Reviews 24-hour/72-hour report from every
unit daily to monitor and ensure timely, effective responses to
significant changes in condition, transfers, discharges, use of
restraints, unexplained injuries, falls, behavioral episodes, and
medication errors. Participate in Quality Assurance and Performance
Improvement (QAPI) initiatives. Support staff education, training,
and competency evaluations. Communicates and clearly documents
any/all changes in resident condition. Provide input and attend
care conferences as needed. Updates care plans as changes occur.
Participates in weekly wound rounds for assigned residents.
Communicates any pertinent unit activities or personnel problems to
appropriate person(s). Participates in shift-to-shift reporting
process per facility policy. Communicates with other departments
effectively. Admits and orients new resident and families and
completes admission paperwork, as assigned. Assist NAR' s and/or
Nurses with bedside care of residents, as needed. Address resident,
family, and staff concerns promptly and professionally. Assist the
Director of Nursing in managing staffing, scheduling, and
departmental budgets. Orders medications from pharmacy per facility
policy. Maintain unit narcotic and controlled drug count records
per facility policy and procedure. Makes necessary arrangements for
discharge or transfer of residents, completes required assessments,
and makes appropriate discharge arrangements. Assists with safety
needs of residents, per facility Safe Patient Handling policies and
procedures. Works with HUC, Care Coordinator, or Nurse Manager on
scheduling of appointments for resident at clinics, offices for
diagnostic, therapeutic, dental, or medical services. Keeps
families/guardians aware of condition and needs of resident. Inform
resident physicians of any unusual or abnormal observations and
completes documentation in resident medical record. Accurately
receives and transcribes physician orders into Matrix Care, per
facility policy. Performs resident exams and treatments per
facility policies and procedures. Accompany physicians/provider on
rounds, as needed. Participate in facility surveys (inspections) as
requested by management. Participates in continuing education
including in-service programs. Assists with staff Performance
Evaluations, as assigned by the Director of Nursing. Rotates weekly
on-call with the nursing leadership team. Participates in Unit,
Quality & Safety Meetings, as directed. Is a resource for staff for
technical needs, such as but not limited to; two-way communication
devices, online records, documentation/ EMAR, email, and web based
continued education. This position will be required to work beyond
normal work hours, additional shifts, weekends, and holidays when
necessary. This role may also require callback during emergencies
(e.g., severe weather, evacuation, or post-disaster). Other duties
as assigned Physical/Mental Requirements Position requires frequent
walking, standing, sitting, bending, stooping, twisting, squatting,
kneeling, and reaching above or at shoulder level. Ability to lift
and/or carry up to 50 pounds occasionally, and up to 35 pounds
frequently, with or without reasonable accommodation. Role may
involve pushing or pulling residents in wheelchairs (300–600 feet)
with resistance equivalent to approximately 35 pounds. Requires the
ability to adapt to changes in daily work hours and schedules. Must
be able to sustain attention and concentration despite frequent
interruptions and manage multiple tasks and messages during an
eight-hour or longer shift. Requires the ability to communicate
effectively in English, both verbally and in writing, with
residents, families, and staff, and to read and understand written
materials, including small print. Reasonable accommodations may be

essential functions of the position. Compensation and Benefits:
Range of 85,000k to 95,000k annually depending of qualifications
and experience. Benefits include medical, dental, 403(b)
