Case Manager ! THIS IS AN HOURLY POSITION, ESTIMATED SALARY BASED
ON HOURLY PAY RATES Pay Rate: $42$45 per hour | Estimated Yearly
Salary: $87,000-93,000k/yr | Work Schedule: Monday-Friday 32-40
hours ( M-F for 40 hours or Tues-Fri for 32 hours ) BrightStar RN

direct deposit Referral program Responsibilities of an RN Case
Manager: No OASIS work! Provides care as defined within the scope
of the states nurse practice act. Makes the initial evaluation
visit and regularly reevaluates the clients nursing needs. Prepares
clinical and progress notes and summaries of care. Serve as backup
for Critical Care Clients, In-School Staffing & PDN Assures proper
maintenance of clinical records in compliance with local, state,
and federal laws. Helps educate and coach care-giving staff and
assist with new hire clinical training, as needed. RN Case Manager
Requirements: Current unrestricted RN license in Minnesota Minimum
two (2) years of clinical experience with one (1) year of
experience in a home care agency, primary care clinic, or health
facility, preferred. Active CPR Certification Negative TB skin test
or QuantiFERON-TB Gold (QFT) blood test (completed within the last
90 days) Reliable car & Auto Insurance Valid Driver's License Clean
background and Criminal Record Understands regulatory compliance
with state and federal regulations. Ability to work on call
