  • A large campus with a dynamic and caring community Job Summary: The ADON works under the Director of Nursing who is accountable for nursing care and services for both assisted living and home care. While the DON brings extensive nursing leadership and care experience within an assisted living setting, the RADON brings nursing leadership and care experience within a home care environment to complement this broader nursing team. While the DON brings strong knowledge of Minnesota's Assisted Living licensure, the ADON brings an expertise with Minnesota's Comprehensive Home Care license In addition, the ADON demonstrates a passion and ability to develop a growing, innovative business modelThe DON and ADON collaborate effectively to ensure the health and safety of all clients through initial and ongoing nursing assessment, care planning and service delivery. The role entails monitoring clients' services, as well as supervising staff, providing staff training and establishing staff competencies prior to delegating care tasks. The successful candidate will be proficient with Eldermark or a similar EHR software. This position requires excellent communication and customer service skills to relate as part of a team and with staff, clients and family members. Essential Functions:
    • Completes initial, face to face comprehensive assessment of prospective/new clients
    • Develops initial service plans based upon assessment results
    • Implements nursing services based upon clients' documented service plans
    • Monitors and reassesses clients, as required and based on changes in clients' needs.
    • Communicates with physicians and other personnel (e.g., case manager or care coordinator) regarding changes in client's needs or conditions
    • Provides direct supervision of new licensed and unlicensed staff within 30 days of beginning employment
    • Directly supervises LPNs' and unlicensed staff members' performance of delegated nursing services
    • Assists the DON, other team members with daily responsibilities as needed
    • Is available for on-call medical questions and/or emergencies via phone as assigned. The 24/7 on-call duties are shared by the DON, RN and LPN
    • Creates the client service schedule and assignments for licensed and unlicensed staff
    • Responsible for assisting the DON in investigating service complaints submitted by clients, clients' representatives or others
    • Other duties assigned Position Type/Expected Hours of Work: Full Time M-F 8am-4:30pm. On-call rotation schedule set by the DON, which can include weekends, evenings and holidays.
      Minimum Qualifications:
      • Must be registered and licensed with the State of Minnesota as a RN, and must maintain current unencumbered license with the Board of Nursing
      • Must have home care nursing experience
      • Mandatory COVID-19 vaccination by 9/1/2021Must demonstrate understanding of Comprehensive Home Care license requirements
        Preferred Qualifications:
        • Nursing leadership experience
        • Experience in an assisted living setting
        • Working knowledge of rules and regulations related to assisted living
